Effective August 12, 2026. NexxstepsRS, LLC ("NexxSteps", "we") operates nexxstepsrs.com and the provider portal at portal.nexxstepsrs.com.
NexxSteps RS is a housing operator. We provide rooms, meals, and transportation at our Phoenix residence to clients placed with us by treatment programs and service agencies ("providers"). We are not a healthcare provider, not a licensed treatment program, and not a HIPAA covered entity or business associate. This policy covers the website and the provider portal.
When an agency applies or uses the portal we collect the organization's legal name, the name, position, email, and phone of the person signing, mailing address, a 24-hour contact for issues involving a client, the levels of care the program provides, and the identifiers a program bills under (NPI, tax ID, taxonomy code, state license, accreditation, Medicaid ID). We also collect certificate-of-insurance details, and a record of the service agreement signed — the version, signer, timestamp, and originating IP address.
To house someone safely we collect what residence staff need: name, date of birth, phone, emergency contact, brief background, special needs (medical, mobility, dietary, accessibility), and any safety flags or restrictions. The placing provider attests that it is authorized to share this information and that any consent required of its program — including under 42 CFR Part 2 where that applies to the provider — is signed and on file with the provider.
We do not collect or store card numbers. Weekly rent is processed by Stripe, which handles card data directly. We retain only a payment reference, the amount, and whether the charge succeeded.
Server logs, IP address, and standard request data. We record IP addresses alongside signatures and acknowledgments as evidence of who agreed to what, and when.
To review provider applications and verify insurance; to house residents safely and coordinate their placement with the placing provider; to charge weekly rent in advance and pursue failed payments; to send the notices our agreements require, including written notice within 24 hours when a client is removed for a House Rules violation; and to keep an audit record of access and changes.
We do not sell personal information, and we do not share it for advertising. We share only:

Access is limited to the placing provider and the residence staff who need it. Data is encrypted in transit and at rest. Every account requires a password, and multi-factor authentication is available. Access and changes are recorded in an append-only audit log.
Resident placement records are kept only as long as needed for the stay and for the records we are required to keep afterwards. Provider and billing records are kept as long as required for tax, accounting, and legal purposes.
Providers can review and correct their organization's information in the portal or by writing to us. A person placed with us, or their provider on their behalf, may ask what we hold about them and ask us to correct it. Write to info@stwos.org.
Our housing serves adults. We do not knowingly collect information from anyone under 18.
If we change this policy we will update the effective date above, and material changes will be communicated to providers through the portal.
